Hours: 15 hours per week over 2 days

Salary: St Luke’s Band 4 £26,677.86-£28,857.35

We are looking for a compassionate, motivated, and skilled Assistant Practitioner (band 4) to join our Day Hospice Team, providing high-quality, person-centred care and support to patients with life-limiting illnesses.

In this rewarding role, you will play a key part in supporting patients and their families with advance care planning, ensuring their wishes and preferences are recognised and respected. You will work closely with the multidisciplinary team to deliver holistic care that promotes comfort, dignity and wellbeing.
