We work closely with Kids First Nurseries that are based within our trust grounds at Wansbeck, North Tyneside and Hexham hospitals to care for our Trust family's children, they offer care for children from 6 weeks old up to 4 years. Job overview

An exciting opportunity has arisen to work in the community heart failure team in North Tyneside as a community heart failure nurse specialist.

We are a small but vital service in caring for patients with a diagnosis of heart failure . The job will involve caring for heart failure patients within a community setting

Main duties of the job

The job involves visiting patients in their own homes, caring for the heart failure patients of North Tyneside, and Northumberland.

There will be cross cover work within the Northumberland community heart failure team. Your role will involve medication titrations, monitoring, support and education of the heart failure patient.

There will be involvement in admission avoidance practices, as well as liaising with the virtual ward team

Detailed job description and main responsibilities

Working in a team, alongside other community Heart Failure Nurse Specialists, you will hold a case load of patients, caring for them in a community setting. You will be involved in titration, education, monitoring and symptom management of the heart failure condition. There will be an element of palliative care too. Liaising with the secondary care Cardiology teams, Palliative care and the wider Primary Care teams.

Qualifications & Registrations

Essential criteria
  • Registered Nurse 1st Level
  • First level degree in health related subject
  • Specialist training to masters level or equivalent level of knowledge and / or experience
  • Mentoring and Teaching qualification E.g. ENB 998 / Registered Mentor
  • Advanced Clinical Skills or equivalent or working towards
  • Independent Nurse Prescriber or working towards

Experience & Knowledge

Essential criteria
  • Significant Post Registration experience
  • Experience in working with patients with Long term conditions and experienced in specialist field E.g Community Cardiology
Desirable criteria
  • Experience of working with patients who have long term conditions in a community setting
  • Experience of working in specialist field e.g Community Cardiology/Heart Failure

Essential criteria
  • It is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, the Trust would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role
